Purple Ribbon Task Force
The Purple Ribbon Task Force was created by the Florida Legislature for a short-term project and was administratively housed within the Department of Elder Affairs. It was tasked with developing a comprehensive state plan to address the needs of individuals with Alzheimer’s Disease and related disorders and their caregivers. The task force consisted of 18 members appointed by the Governor, President of the Florida Senate, and Speaker of the Florida House of Representatives. The task force submitted its final report and recommendations for an Alzheimer’s Disease state strategy to the Governor and the Legislature on August 1, 2013. The task force adjourned following that submission.
New 2014 Caregiver Survey
This survey will be closed effective January 9, 2015.
Thank you to the over 1000 Florida caregivers of persons with Alzheimer's and related dementias who have provided their valuable insights in their challenges and needs.
The Department of Elder Affairs is conducting a statewide survey of caregivers of persons with Alzheimer’s disease and related disorders (ADRD). The survey will collect information on caregiving conditions in Florida, such as the need for services or support for persons with ADRD and their caregivers; their knowledge about the availability of services; the impact of caregiving on caregivers and others; and the continued needs for services across the state.
The survey was developed in response to a recommendation of the Purple Ribbon Task Force, and it builds on the set of surveys previously conducted by the Task Force in 2013. The results of this new survey will further assist the Department to assess the needs of persons with ADRD and their caregivers.
The survey can be taken in either of two ways. It can be answered online, or a printed version can be completed and mailed or faxed to the Department. The survey is available in English and Spanish online at the following links:Â

A flyer with information about the caregiver survey and tear-off tabs containing the web address of the survey is also available. Flyers can be distributed to caregivers of individuals with ADRD or left in public places, such as supermarkets, which caregivers might visit.
