The Department of Elder Affairs welcomes your online subscription to the Elder Update newspaper.
How to use the subscription form below:
Simply type in your responses to information requested. Use the TAB key to move to the next field. If you need to move back to the previous field, use SHIFT-TAB or click on the field with the mouse. Fields in BOLDFACE type are required to process the subscription request and must be filled in. Once all of the required fields are filled in, click on the SEND button at the bottom of the form to send your request.
If all responses are filled in correctly, you will receive a "Thank You" message acknowledging that your subscription request has been sent and will be processed. If you will receive an "Error" message, please return to the subscription request form by clicking on the BACK button, complete the required information and send it again.
*Please allow 8 to 10 weeks to receive the first issue or for changes to take effect.
